We know that the source of The Doctor's survival is the red crystal after his body is destroyed.
After the Core is destroyed, we don't see the crystal breaking.
If the crystal did survive then The Doctor's consciousness would still be intact.

I believe if The Doctor did survive, he would have lost all his powers (as Ballos gets killed making the Demon Crown cease to exist) and would have the form of a ghost.
However he would still be able to think and probably communicate.

Yeah, that always kinda bothered me. I was never really satisfied with either of the good endings, storyline-wise...

Well I thought all the endings were good except for the bad one.
Quote escaping the island and spending the rest of his life living with Kazuma.....scary.

Possibly while Quote and Sue were escaping, The Doctor's consciousness slipped away with the crystal.

I think he is dead.
We know a few things from the game:
- Destroying the core will turn the mimigas back to normal
- The mimigas are frenzied because of the red flowers
- The red crystal is refined red flowers
- The red crystal keeps the doctor alive after his body is destroyed.

Thus we can make the following conclusions:
- The power of the red flowers and crystal is destroyed along with the core
- The doctor is entirely reliant on the red crystal
- The Doctor, in his non-corporeal form, will also be destroyed along with the core

The core is most certainly destroyed, therefore the doctor is dead.

But don't let me spoil the party.

To be honest, I too think The Doctor perished when the Core was destroyed.
As you said the power of the red flowers would be destroyed along with the core but that does not necessarily mean that the red crystal would break too.

The Doctor would still have the Demon Crown in consciousness form meaning he might have used it to survive or at least preserve himself.
But then he would die when Ballos gets killed.

This is all a bit ambiguous so I'll see what everyone else has to say before making a conclusion.

Pixel likes leaving a lot things for the player's imagination. I've never given much attention to the Doctor's death though, perhaps he really did survive though. Maybe he's just a ghost of some sort.

The Demon Crown is a physical object, albeit one which comes back when destroyed, and it appeared to be temporarily destroyed when the Doctor was defeated by Quote. Hence why Misery could rebel against him in Black Space - he did not have the Demon Crown.

As has been said, destroying the Core undoes the effects of the red flowers, and the Red Crystal is merely purified red flowers, so its power would also have been undone. Hence why Sue reverted to normal (as she was turned monstrous by the Red Crystal).

The Doctor's power was removed, and the body he was inhabiting (the Undead Core) was also destroyed, so he's deader than dead.

As for why the island stops falling in the perfect ending, it could well have been Misery holding it up, or possibly Ballos's soul and magic permeates the island and holds it up after he's killed.

The theory in A Lost Land mod, I think, is that since Ballos did want to die in order to destroy his magic, he believed that the island falling would have killed him. The Core was there in order to nullify this wish, so when the Core was destroyed and Ballos wasn't, the island was falling. In the good ending, after you kill Ballos, the island doesn't fall ... soo ...
